The I-Port™ is a prescription only device. If you think that you can benefit from using the I-Port™, print the Prescription Order Form and take it with you when you speak to your healthcare professional.

Once the Prescription Order Form has been completed and signed by you and your healthcare professional, fax the form to Patton Medical Devices at 1-866-286-1401 and a Customer Service Associate will contact you within 48 hours to discuss the status of your order.
